  • $99 for Patients on SSA or VA Disability or Government Assistance
  • $199 Veterans (must provide proof - DD214 or VA card)
  • $299 All Other Patients
  • $25 Replacement of Expired Certification

Qualifying Conditions
  • MUST be a Missouri resident and have a valid Missouri ID
  • MUST have medical records to see one of our physicians.

A doctor’s letter is not sufficient. You must provide medical records that show a diagnosis of one or more of the qualifying conditions. If you have any questions, please do not hesitate to contact our office.

No Show Policy

If you are not present for your appointment within 15 minutes of your scheduled appointment, we will need to reschedule your appointment. To reschedule, we will need you to contact our office PRIOR to your appointment time to reschedule or you may reschedule your appointment through the secure portal online (please see the email you received from our office when you made your appointment).
